Fatty liver, metabolic health and liver function

This is the one on our site with genuinely good news: fatty liver is reversible.

Not managed — reversed. Sustained weight loss of around 7–10% of body weight clears fat from the liver in a large proportion of people, and can improve inflammation too. That is a real, well-evidenced outcome, and it is achievable without anything exotic.

What it is, and why almost nobody has symptoms

Fat accumulates inside liver cells. Now usually called MASLD (metabolic dysfunction-associated steatotic liver disease), previously NAFLD. It affects roughly a third of Indian adults, and it is closely tied to insulin resistance — which is why it travels with abdominal weight, high triglycerides and raised blood sugar.

Most people have no symptoms at all until it is advanced. It is usually found incidentally on an ultrasound done for something else, or through mildly abnormal liver enzymes. The absence of symptoms is not reassurance.

The concern is the minority in whom it progresses to inflammation (steatohepatitis), then fibrosis, then cirrhosis. That path takes years and is interruptible — which is the whole argument for acting early.

The FIB-4 is the one to insist on. Grade of fat on ultrasound matters far less than whether there is fibrosis, and FIB-4 costs nothing beyond blood tests you probably already have.


The alcohol question, asked plainly

If your intake is more than roughly 20g of alcohol a day for men or 10g for women, this is not metabolic fatty liver — it is alcohol-related liver disease, and the treatment is different and more urgent.

We will ask directly and without judgement. An inaccurate answer produces an inaccurate treatment plan, and this is the one question where that matters most.

An uncomfortable point about herbal liver products

Herb-induced liver injury is real, and Ayurvedic and herbal supplements are among the recognised causes in India. Some products marketed for liver health have been found to contain heavy metals. Others contain concentrated extracts at doses no classical text would recognise.

A liver that is already inflamed is the worst possible organ on which to experiment with an unlabelled powder from an unverified source.

If you take anything from this page, take this: ask your supplier for a batch heavy-metals report. If they cannot produce one, do not put it in your liver.


When to seek urgent care

  • Yellowing of the eyes or skin
  • Abdominal swelling, or swelling of the legs
  • Vomiting blood, or black tarry stools — emergency, go now
  • Confusion or marked drowsiness
  • Severe persistent right upper abdominal pain

You should be under a hepatologist, not us alone, if you have diagnosed fibrosis or cirrhosis, hepatitis B or C, or enzymes more than three times the upper limit of normal.


Questions people actually ask

Can fatty liver be reversed? Grade I and II frequently reverse with sustained weight loss and metabolic correction. Established fibrosis is a different matter and needs specialist care. Get a FIB-4 so you know which conversation you are in.

Can a herbal formulation alone reverse it? No. Weight and metabolic change do the work. A formulation supports that process and addresses digestion and the constitution alongside it.

How long? Reassessed at 12 weeks, with repeat enzymes. Ultrasound changes take longer — usually six months or more.

Do I need to stop alcohol completely? If there is any liver involvement, yes, at least while it is being corrected.
