Type 2 Diabetes Reversal

Type 2 diabetes can be reversed. The research that proved it also shows exactly how — and that is what our program is built on.

For decades, type 2 diabetes was treated as permanent and progressive: more tablets every year, then insulin. Research from Yale, Newcastle University and a large UK trial changed that picture. In many people it can be put into reversal — normal blood sugar, off medication — and the mechanism is now well understood.

The research

Yale University (2005): it starts in the liver

Gerald Shulman's group at Yale studied patients with poorly controlled type 2 diabetes. Weight loss of less than 10% of body weight produced a marked fall in liver fat, restored the liver's response to insulin, and normalised fasting blood sugar. The paper's title says it plainly: Reversal of … hyperglycemia by moderate weight reduction.

Newcastle University (2011): the pancreas recovers too

Professor Roy Taylor's team put 11 people with type 2 diabetes on a 600 kcal/day diet for eight weeks. Fasting blood sugar normalised within the first week. By week eight, the pancreas's insulin-producing (beta-cell) function had returned to normal, alongside falling fat in both the liver and the pancreas. Their title: Reversal of type 2 diabetes.

Taylor's explanation, the twin cycle, is the mechanism in two lines: excess fat in the liver drives insulin resistance; fat spilling into the pancreas suppresses insulin production. Remove the fat from both organs, and both recover.

The DiRECT trial (2018–2024): it works in ordinary clinics

Run through GP practices in the UK: a formula diet for 12–20 weeks, medication stopped at the start under medical supervision, then structured food reintroduction.

46% reversed at one year, 36% at two years, and some held it for five years. The result tracked weight loss almost exactly:

What "reversal" means in our program

We use the international definition agreed in 2021 by the American Diabetes Association, EASD, Diabetes UK and the Endocrine Society:

HbA1c below 6.5%, sustained for at least three months, off all glucose-lowering medication.

That is what we measure and what we report back to you. Those bodies call this state remission; the research papers above call it reversal. They describe the same result. What it does not mean is "cured forever" — see the maintenance phase below.


Who it works best for

  • Type 2 diabetes only. Type 1 diabetes and LADA are autoimmune and cannot be reversed by diet. If there is any doubt about your type, we test before we start.
  • Shorter duration. The sooner after diagnosis, the more likely the pancreas recovers fully.
  • People able to lose 10–15 kg. The DiRECT table above is the honest guide.
  • Long-standing diabetes or years on insulin: full reversal is less likely, but lower HbA1c and fewer medications usually are not. We will tell you which is realistic at your assessment.

The program

Phase 1 — Assessment (week 0)

  • HbA1c, fasting glucose and fasting insulin (HOMA-IR)
  • Lipid profile
  • Liver enzymes and FIB-4 — the liver is the organ this program targets
  • Kidney function: creatinine, eGFR, urine albumin–creatinine ratio
  • C-peptide and GAD antibodies if your diabetes type is uncertain
  • Full medication review, coordinated with your treating doctor
  • Prakriti and agni assessment by our registered physician

Phase 2 — Weight loss (weeks 1–12)

A structured, calorie-controlled eating plan built around your constitution and digestion, targeting 10–15 kg. Home glucose monitoring throughout. Medication is reduced by your treating doctor as your numbers fall — never by us, and never by you. Weekly check-ins.

Phase 3 — Reintroduction (weeks 13–20)

Food reintroduced in stages, watching glucose response, so you learn what your body tolerates.

Phase 4 — Maintenance (month 6 onwards)

Reversal lasts as long as the weight stays off. If it comes back, so does the diabetes. Quarterly HbA1c, and continued support. This is where most programs fail, and where most of our effort goes.


Where Ayurveda fits — honestly

The research is clear about what reverses type 2 diabetes: losing weight, and with it the fat in the liver and pancreas. No herb has been shown to reverse diabetes on its own, including the ones we prescribe.

What Ayurvedic care does is make the weight loss achievable and the result durable:

  • Cravings and appetite — the main reason weight comes back
  • Sleep — poor sleep raises insulin resistance directly

Medication safety — read this before you start

When you eat much less, the medication you already take can push your blood sugar too low. DiRECT stopped glucose-lowering and blood-pressure medication on day one, under GP supervision, for exactly this reason.

  • **Never start the weight-loss phase without your treating doctor reviewing your medication.** We will write to your doctor with your plan.
  • Sulfonylureas (glimepiride, gliclazide, glibenclamide) and insulin carry the highest risk of hypoglycaemia on a reduced diet.
  • SGLT2 inhibitors (dapagliflozin, empagliflozin) combined with very low calorie or low carbohydrate intake can cause ketoacidosis even when blood sugar looks normal. Your doctor needs to know your plan.
  • Blood-pressure medication often needs reducing as weight falls — dizziness on standing is the sign.
  • Some Ayurvedic herbs also lower blood sugar. Their effect adds to your medication's — which is why your doctor adjusts your plan as you go.
  • Know the signs of low blood sugar — shakiness, sweating, confusion, palpitations — and keep glucose tablets with you.

When to seek urgent care

  • Blood sugar above 250 mg/dL with vomiting, abdominal pain or laboured breathing
  • Symptoms of low blood sugar that do not resolve with glucose
  • A foot ulcer or wound that is not healing
  • Sudden change in vision
  • Chest pain or breathlessness

These are emergency-room problems. Go.


Questions people actually ask

Is reversal permanent? It lasts as long as the weight stays off. Regain the weight and the diabetes usually returns — which is why the maintenance phase is part of the program, not an optional extra.

Will your herbs reverse my diabetes? No. Weight loss, and the fall in liver and pancreas fat that comes with it, is what the research shows reverses type 2 diabetes. Our formulations support digestion, appetite, sleep and stress — which is often what makes the weight loss possible.

Can I stop metformin or insulin when I join? Only your treating doctor can decide that, and on a program like this they usually will reduce it, in steps, as your numbers fall.

How long does it take? The weight-loss phase is 12 weeks. Reversal is confirmed only after at least three months off medication with HbA1c below 6.5%.

I have had diabetes for 15 years. Is it worth trying? Usually yes — even when full reversal is unlikely, most people come out with a lower HbA1c and fewer medications. We will be straight with you at the assessment about which outcome is realistic.

What about type 1 diabetes? Type 1 is autoimmune and cannot be reversed by diet or herbs. Please do not reduce insulin on the strength of anything written here.
